We can provide a sick note for school. If you need a school physical form, sports clearance, or a camp form please contact your nearest Pediatrician's office as your child will need an in person physical exam.

Also nausea, vomiting, diarrhea, bloody stools, viral illnesses, congestion, feeding issues, weight concerns (under weight or overweight). Nightmares, bed wetting, sleep difficulties. Headaches, and Learning difficulties. Behavior problems at home and in school. Depression, and mood problems. Important to note that Controlled medications like the ones prescribed for ADHD cannot be prescribed through telemedicine.
